This Care Collective Membership Agreement ("Agreement") governs your membership in the Care Collective program offered by Dementia Careblazers LLC ("Careblazers," "we," "us," or "our"). By enrolling in the Care Collective, you agree to be bound by this Agreement and by our general Terms and Conditions at careblazers.com/terms-and-conditions. In the event of a conflict between this Agreement and the general Terms and Conditions, this Agreement controls for Care Collective matters.
The Care Collective is a membership program for family dementia caregivers that includes:
Support rooms are facilitated by community moderators. Dr. Natali Edmonds may participate or provide coverage when a regular moderator is unavailable. Support rooms are not clinical group therapy and do not establish a treatment relationship between participants and Dementia Careblazers or Dr. Natali Edmonds.
The Care Collective was previously offered under the names Dementia Care Club, Care Course, and Dementia Care Course. Members who enrolled under any of these prior names are subject to this Agreement.
The content, schedule, and format of Care Collective offerings may change from time to time. We reserve the right to modify, add, or discontinue any program component with at least 14 days' notice. Changes to core program components will be communicated to active members in advance.

The Care Collective is offered on a monthly subscription basis. By enrolling, you authorize Dementia Careblazers to charge your payment method on a recurring basis as described below. You may cancel your monthly subscription at any time as described in Section 4.
Monthly Membership — $59.00 per month
Monthly members are charged $59.00 every 30 days from the date of enrollment, automatically and on a recurring basis, until you cancel. Your charge date recurs on or about the same calendar day each month.
This is a recurring subscription. Your payment method will be charged automatically each billing period until you cancel. See Section 4 for how to cancel.
Annual Access
The annual access option is a single one-time payment for 12 months of Care Collective access. The standard annual access price is $697.00. Promotional pricing may be offered at various times throughout the year at a lower rate. The price you pay at enrollment is your annual access fee for that 12-month period, regardless of any promotional rates offered before or after your enrollment date.
Annual access does not automatically renew. Your access will expire 12 months from your enrollment date. Prior to expiration, you will receive renewal emails with the option to re-enroll. See Section 3 for renewal options and pricing.
Annual access members may choose to renew into a monthly or annual renewal plan when their current access expires. See Section 3 for details.
Dementia Careblazers reserves the right to modify membership pricing. We will provide at least 30 days notice of any price change before it takes effect for existing members with active subscriptions. Continued membership after notice of a price change constitutes acceptance of the new rate.
This Agreement is intended to comply with applicable federal and state automatic renewal laws. Members located in states with specific automatic renewal disclosure or notice requirements may receive additional disclosures as required by applicable law.
↑ Back to topYour annual access membership expires 12 months from your enrollment date. Dementia Careblazers will send you a renewal reminder by email at least 30 days before your membership expires, containing a link to renew.
At renewal, you may choose between two recurring options:
Annual Renewal — $464.00 per year
Billed automatically each year on your renewal date.
Recurring annual subscription. Cancel before your renewal date to stop future charges.
Monthly Renewal — $47.00 per month
Billed automatically each month on your renewal date.
Recurring monthly subscription. Cancel anytime to stop future charges.
Both renewal options are recurring, automatic-payment subscriptions. Neither is eligible for a refund. The 60-Day Feel Better Guarantee applies only to initial annual access purchases and does not apply to renewal subscriptions.
Renewal pricing is available to members completing a 12-month annual access period. It is separate from new member pricing and from discounted annual pay-in-full offers that may be available throughout the year; those discounted prices are for initial access only.
If you do not renew before your expiration date, all Care Collective access will end, including access to the member community, live sessions, training library, and session replays.
Members with an active renewal subscription may manage their plan through the Stripe billing portal, accessible in the Care Collective member area.
If you would like to switch between annual and monthly renewal (in either direction), contact us at [email protected]. The switch will take effect at the end of your current paid renewal period.
$59.00/month members are on an ongoing monthly subscription and are not eligible for the $47.00/month renewal pricing. The renewal rate is available only to members completing a 12-month annual access period.
If you are a $59.00/month member and would like to switch to an annual pay-in-full plan, contact us at [email protected]. Your 12-month access period will begin on the day of the switch.
After 365 consecutive days of active monthly membership, you may request to switch to the $464.00/year annual renewal plan. Contact us at [email protected] and we will provide a renewal link at that time.
↑ Back to topMonthly members may cancel their subscription at any time using either of the following methods:
In both cases, cancellation takes effect at the end of your current billing period. You will retain full access to the Care Collective through the last day of the period for which you have paid. You will not be charged for the following billing period.
No prorated refunds are issued for cancellations made mid-billing period. Monthly members may cancel anytime without penalty.
Annual access is a one-time purchase and does not renew automatically. There is no cancellation process for annual access; your access simply expires at the end of the 12-month period. You are not required to take any action to stop being charged; no further charges will occur unless you choose to re-enroll.

↑ Back to topMonthly memberships are not eligible for refunds. Monthly members pay one month at a time and may cancel at any time before their next billing date. There is no long-term financial commitment, and no refund will be issued for the current billing period after it has begun.
Annual access members are protected by our 60-Day Feel Better Guarantee. To qualify for a refund under this guarantee:
If you meet the above conditions and request a refund within your first 60 days, we will issue a full refund of the annual membership fee you paid. No processing fees will be deducted from guarantee refunds.
After 60 days, annual access payments are final and non-refundable. Annual access members who cancel access before the 60-day window without meeting the participation requirements are not eligible for a refund.
What "full refund" means: We will refund the annual membership fee you paid ($697 or the discounted price you paid at enrollment). We will not deduct credit card processing fees from the refund amount.
Renewal subscriptions (annual at $464.00/year and monthly at $47.00/month) are not eligible for refunds. See Section 3 for renewal plan details.
↑ Back to topMembers who originally enrolled under the Dementia Care Club ("DCC") subscription model remain on their original pricing and billing terms unless they have re-enrolled under the current Care Collective structure.
DCC Monthly — $47.00 per month
Legacy DCC monthly members are billed $47.00 automatically each month. To cancel, use the self-service Stripe billing portal in the member area. If you are unable to access the portal, email [email protected] and we will process your cancellation before your next billing date. Cancellation takes effect at the end of your current billing period.
Important: Auto-Renewing Subscription
Legacy DCC annual members are billed $464.00 automatically each year on or about the anniversary of their enrollment date. Your payment method will be charged $464.00 each year unless you cancel before your renewal date.
Renewal Notice: Dementia Careblazers will send you a renewal reminder by email at least 30 days before your next annual charge.
To cancel your DCC annual subscription and stop future billing, use the self-service Stripe billing portal in the member area. If you are unable to access the portal, email [email protected] and we will process your cancellation before your next billing date. Cancellation takes effect at the end of your current annual period; you will retain access through the end of the paid year.
Legacy DCC members are subject to this Agreement in all other respects, including the community conduct, confidentiality, intellectual property, and dispute resolution provisions of the general Terms and Conditions.
↑ Back to topLive support rooms, Q&A calls, and other live sessions are offered on scheduled dates. Dementia Careblazers does not guarantee that you will be able to attend every live session. Replays of eligible sessions are made available to active members in the member area, subject to availability.
We reserve the right to reschedule, cancel, or modify any live session. In the event of a cancellation by Dementia Careblazers, we will make reasonable efforts to reschedule or provide a recorded alternative. We are not liable for any inconvenience caused by schedule changes.
Missing a live session does not constitute grounds for a refund under any circumstances, including under the 60-Day Feel Better Guarantee.
↑ Back to topYour participation in the Care Collective community is subject to the Community Conduct policy set forth in the general Terms and Conditions at careblazers.com/terms-and-conditions. Violations of that policy may result in immediate removal from the Care Collective without refund.
The Care Collective community — including support rooms, live Q&A sessions, live classes, and the Circle community forum — is a space where members share personal and sensitive information about their caregiving experiences. To protect the privacy and dignity of all members:
Violations of these provisions may result in immediate removal from the Care Collective without refund. Violations may also expose the member to civil liability under applicable law. Unauthorized reproduction or distribution of Care Collective materials may constitute copyright infringement and may be subject to legal action.
Members may not use the Care Collective community, Circle platform, member directory, or contact information of other members to:
Members are welcome to build genuine friendships and connections with other Careblazers. This provision is not intended to restrict personal relationships; it applies to organized efforts to redirect members away from the Care Collective community.
Violations may result in immediate removal from the Care Collective without refund and may be subject to legal action.

↑ Back to topYour Care Collective access is personal to you and non-transferable. You may not share your login credentials with any other person.
Dementia Careblazers reserves the right to terminate or suspend your membership at any time for violations of this Agreement or the general Terms and Conditions, including for conduct violations, payment failures, or fraudulent chargebacks or chargebacks filed in bad faith after our dispute resolution and refund process was made available to you. Termination for cause does not entitle you to a refund.
↑ Back to topThis Agreement is part of and subject to the general Terms and Conditions at careblazers.com/terms-and-conditions, including the Medical and Professional Disclaimer, Limitation of Liability, Indemnification, Dispute Resolution, and all other provisions that apply to members and users. By enrolling in the Care Collective, you confirm that you have read and agree to both this Agreement and the general Terms and Conditions.
This Agreement is governed by the laws of the State of Arizona, United States, without regard to conflict of law principles.
Careblazers' liability to you in connection with your membership is limited as described in the Limitation of Liability section of the general Terms and Conditions at careblazers.com/terms-and-conditions.
Any disputes arising under this Agreement are subject to the dispute resolution process set forth in the general Terms and Conditions at careblazers.com/terms-and-conditions.
Participation in the Care Collective — including attending live Q&A sessions, support rooms, or live classes; accessing workbooks, training materials, or recorded content; or any other interaction with Dr. Natali Edmonds through the program — does not create a psychologist-client, therapist-patient, or any other professional or clinical relationship. Dr. Natali Edmonds is not acting as your psychologist, therapist, or clinical provider through the Care Collective. If you need clinical support, please consult a licensed mental health professional in your area.
The Care Collective is an educational membership program. Content provided through the Care Collective, including information shared by Dr. Natali Edmonds, is educational in nature and does not constitute medical, psychological, or clinical advice. See the Medical and Professional Disclaimer in the general Terms and Conditions for full details.
All Care Collective content — including recordings, session replays, live classes, written materials, workbooks, and training resources — is owned by Dementia Careblazers LLC and protected by copyright. Your membership gives you a personal, non-transferable license to access this content during your active membership period. See Section 8 for member confidentiality and distribution restrictions.
The Care Collective community is hosted on Circle (careblazers.circle.so). By participating in the Circle community, you are also subject to Circle's Terms of Service (circle.so/terms). Dementia Careblazers is not responsible for Circle's platform policies or availability.
Dementia Careblazers makes reasonable efforts to maintain consistent access to Care Collective resources and live sessions. However, we are not liable for service interruptions caused by third-party platform outages (including Circle, Zoom, or Stripe), technical issues beyond our reasonable control, or other circumstances beyond our reasonable control. Service interruptions do not constitute grounds for a refund.

We process your membership data on the basis of contractual necessity — your personal information is required to provide and administer your Care Collective membership.
If you are located in the European Union or European Economic Area, you have the following rights under the General Data Protection Regulation (GDPR):

You also have the right to lodge a complaint with the data protection supervisory authority in your country of residence.
